Please read these notes before completing the team sheet as they contain the details of where the team sheet must be sent and by when

Team sheets must be correctly completed and submitted to the Assistant Registration Secretary (Junior Division) or the Small Sided Co-ordinator (Ladies Division) within 72 hours of the fixture being played. Teams will be penalised if they fail to comply to these requirements.
